It is with a heavy heart that we share the passing of our dear colleague and friend, Consuelo Hines. Consuelo was a beloved member of the IPS and CP&S team since 2016, and everyone who had the privilege of knowing her will feel this loss deeply.


Consuelo's teammates often referred to her as the "office sweetheart" or "sunshine," a testament to the warmth and positivity she radiated. Her teammates will keep the memory of her unwavering kindness, genuine smile, and caring nature close to their hearts.


One example of Consuelo's warmheartedness is her daily good morning text, which she would send to an ever-growing list. She also never missed an opportunity to share with her colleagues just how much she cherished them. Her actions were the embodiment of love and generosity, making her a source of inspiration for us all.


Consuelo loved her coworkers like a family, and her impact on our team will be a lasting legacy. Let's honor Consuelo's memory by embracing the love and kindness she so effortlessly shared with us.


Please keep our friends in Bowling Green, Glasgow, and Franklin in your thoughts. We are wishing comfort and strength to all.

An Update on PTO


I acknowledge that we started 2024 on the wrong foot with PTO account balances. For this, we are very sorry. For sure, not our intention.

A Letter from Our Director of Complete Care Services


As the leader of Complete Care Services, I am very excited to share some additional information and updates about our new, innovative, and patient-focused Complete Care Program. Complete Care is a Commonwealth Pain & Spine branded initiative that combines existing services into one bundled program to better address a comprehensive and personalized approach to our patients’ health and wellness, especially related to chronic pain. This program aligns with Commonwealth’s vision and provides our patients with a wide-ranging toolbox of vital services, including Chronic Care Management (CCM) services.


I am pleased to announce that we have officially launched CCM services at our IPS locations and have been enrolling patients into the program. We are working hard to continue growing our team to expand and provide these services across the company, ensuring we offer this important service to all eligible patients. As we grow, we will be able to provide a Medical Care Coach as a companion to assist in coordinating care across the healthcare continuum for all eligible CP&S patients.

 

You will soon begin to see the Complete Care logo on our website and marketing materials – your support is crucial as we build and promote this program. I look forward to visiting our clinics over the next few months and further introducing our team to Complete Care.

Open Enrollment Update


We've been busy during January conducting audits and partnering with carriers following our open enrollment and changes that were effective on 1/1/2024.

It's time to share an update.

 

  • Medical plans - Anthem's system has been updated with all plan changes and ID cards have been printed.  

  • Anthem - Sydney App - if you haven't downloaded it to access all of your medical information, including your ID card, claims, provider information, and health tips - please take a few minutes and do so today.

  • HSA and/or FSA - NEW CARRIER​ - HSA Bank - all cards have been mailed.  If you've not received your card at your home address, please send an email to AskHr.

  • HSA - 2023 carrier was Voya - if you had a remaining balance in your Voya account, you may leave it there OR you may transfer it to HSA Bank.  Instructions can be found by clicking here.

  • Delta Dental - system is updated. No cards will be mailed.  If you need a card, you may print one from www.deltadental-ky.com

  • Vision - system is updated. No cards will mailed. Coverage can be accessed via Sydney app or online at Anthem.com.

It's tax season once again. Your 2023 W2 is available via Paylocity.


How do you find your W2?

  • Log into Paylocity

  • Click on Self Service Portal 

  • Click on Pay

  • Click on More (to find more menu items)

  • Click on Tax Forms

  • Click on 2023

  • Click on download (top right corner)

 

IMPORTANT NOTE: Bowling Green, Glasgow, and Franklin team members, please remember that you will receive your 2023 W2s from Paycor.
